About Course

The purpose of this NEW 2-hour webinar is to provide the attendee with a variety of home-based exercises for the homebound patient. The home health setting is unique in that the clinician must learn to adjust and adapt the HEP to the setting and the patient. The course will review essential exercise principles such as open-chain, closed-chain, gravity-reduced, against-gravity, RPEbased, as well as others. Additionally, outcome measures such as 6MWT, TUG, 5-times STS, 30-sec STS, BBS, DGI, FES, etc. will be interwoven into treatment session. And lastly, attendees will be presented with four very different case-studies and watch and listen while the instructor develops and delivers individualized ‘task-specific and function-based’ interventions. The course will showcase how the treating therapist can apply "task-specific, function-based" exercise program. Learn home programs that take place in the kitchen, living room, bedroom, and bathroom, in the garage and outside on patios and in yards. These exercises apply to patients with orthopedic problems, neurologic problems, cardiovascular problems, and Oncologic problems. The material presented will be within the parameters of the Conditions of participation (CoP) under Centers of Medicare and Medicaid Services (CMS).

PRESENTERS

Patrick A. Tino, PT, DPT first earned his AAS in Liberal Arts and Physical Therapist Assistant studies in 1993 from SUNY Broome College in Binghamton, NY. He’s gone on to earn a Bachelors in Biological Sciences from Barry University in Miami, Florida in 1996 and Masters and Doctorate degrees in Physical Therapy from Nova Southeastern University in Ft. Lauderdale, Florida in 1998 and 2010 respectively. Patrick’s home health experience dates back to 1993 during the “Fee for Service” days and moves through the “Perspective Payment System” PPS days and continues presently to the “Patient-Driven Groupings Model” PDGM system. He has successfully navigated the tumultuous landscape at every turn during the past 27 years practicing in 4 states. He has delivered lectures on the topic of home health therapy to numerous students over his career. He lives in Tampa Bay, Florida with his wife, 3 children, and 2 dogs.

OBJECTIVES

Upon completion of this course, the participant will be able to:

1) Identify and describe the timeframes for the various payment systems in Home Health.

2) Summarize to your students, patients, and insurance company reviewers what is “medically necessary” and “reasonable and justified” to support the provisions of therapy services within the home under the conditions of participation “CoPs”

3) Choose relevant and specific tests and measures that examine and evaluate “functional level” of a patient within the home setting.

4) Design several different types of home programs that include “task-specific, ADL-related functional movements where the patient is integrating and addressing the all-important OASIS data-points of: transferring, ambulating, toileting, dressing, showering, and eating.

5) Critically and objectively analyze and appraise the all-important social dynamic between the patient and in-home caregiver(s)/family members when determining plan of care and writing goals as a measure of achieving a good functional outcome.
